If you are retired from Delta Air Lines and over age 65 and would like to enrolled in the DALRC Plans, you need to:

1) Print off the Group Retiree Insurance Enrollment Form (fill out all information on the form)

2) Print off the applicable Enrollment Election Worksheet (check the coverages you want)

3) Mail the information to:
